Army Community Service

Army Community Service (ACS) Provides a wide range of programs dedicated to Soldiers, family members and all other authorized personnel. The offices are staffed by civilian personnel and dedicated volunteers at Dickinson Hall at 615 McClellan Ave.
Open Monday through Friday, 7:30 a.m. to 4:30 p.m., call for information at 684-2800 or 684-HELP (684-4357).

Army Emergency Relief (AER) is managed by ACS in Dickinson Hall on McClellan Avenue. AER gives emergency financial assistance to Soldiers, retirees and family members. It is an independent, nonprofit organization that offers no-interest loans and grants. AER can also help service members from other military components, with the approval of their branch relief organization. Call 684-2800 for detailed information.
Information and Referral (I&R) provides answers and referral assistance for all needs. I&R also distributes phone books, Welcome Guides and information on new and upcoming activities on the installation.

ACS staff members are available to help with information on English as a Second Language (ESL) classes and the Newcomers’ Orientations for service members and families. Visit the Loan Closet between 8 a.m. and 4 p.m., Monday through Friday, for short-term loans of basic living items.

Relocation Assistance Program (RAP) helps people handle the mobile military lifestyle. Visit www.militaryhomefront.dod.mil to get instant access to information on worldwide military installations.

Financial Readiness Program (FRP) helps Soldiers and families with budget counseling, checking account management, consumer education and classes to keep Soldiers updated on all new DoD financial programs. Call ACS for an appointment. The Food Locker distributes non-perishable food items and food vouchers to service members and families in times of need.

ACS Volunteer Program Opportunities include serving as a village mayor or vice-mayor, providing administrative assistance or information and referral services in the ACS office, teaching English as Second Language, and working in almost every other ACS program. Reimbursement for child care costs is available to ACS volunteers.

Installation Mayoral Program Installation mayors are key volunteers who serve as a liaison between residents and senior leaders on quality of life issues. Each installation housing area (village) has a mayor and/or vice mayor who attends a monthly meeting, disseminates information to residents, and serves as an important point of contact and advocate for their community. Reimbursement for child care costs is available and all mayors and vice mayors are provided with a designated parking permit.

The Family Advocacy Program (FAP) is the Army’s program to prevent abuse be providing a variety of services to strengthen Army families and enhance their ability to adapt to military life. FAP is dedicated to the prevention, education, prompt and timely reporting, investigation, intervention and treatment of domestic and child abuse through rehabilitation and treatment.

Although FAP coordinates the legal, law enforcement, social services and medical aspects of family violence on each installation, program responsibilities are primarily divided at the installation between ACS and medical treatment facility staffs. Families wishing treatment and counseling can request restricted reporting by contacting a health care provider, the FAP manager or a victim advocate. They will be offered medical care and counseling but an investigation or legal action is not initiated. Unrestricted reporting is reporting domestic violence to any other person than the FAP manager, a health care provider, chaplain or victim advocate.

The Sexual Assault Prevention and Response Program (SAPRP) reinforces the Army’s commitment to eliminate incidents of sexual assault through a comprehensive policy that centers on awareness and prevention, training and education, victim advocacy, response, reporting and follow up. Army policy promotes sensitive care and confidential reporting for victims of sexual assault and accountability for those who commit these crimes. For the purposes of this policy, confidentiality or confidential reporting is defined as allowing a Soldier to report a sexual assault to specified individuals. This reporting option gives the Soldier access to medical care, counseling and victim advocacy without initiating the investigative process.

There are two types of reporting. Restricted reporting provides for medical care and counseling — there is no investigation or command involvement at this level. For restricted reporting, contact a Sexual Assault Response Coordinator (SARC), the installation victim advocate,
a unit victim advocate or a health care provider. Unrestricted reporting can be made to anyone and the victim will be provided medical treatment, counseling, a forensic exam, a CID investigation, victim witness liaison contact and legal support. The Victim Advocate Program (VAP) coordinator provides advocacy for families involved in domestic violence or sexual assault.

The Parent to Parent (PTP) program provides bonding/parenting information for new parents, home visits and intervention programs to families at risk for abuse. The program helps new and first-time parents. Services are available to all parents from pregnancy through the teen years. The program is designed to strengthen parents’ knowledge and skills so they can provide environments where their children can thrive. The program also seeks to reduce stressors that can increase the risk of child abuse and neglect. Weekly playgroups are provided for preschoolers and their parents from August through May.

The Exceptional Family Member Program (EFMP) provides information and resource referrals to military and civilian community support agencies. The EFMP manager offers advocacy support and can help families with special education issues, including assessments, evaluations, Individual Family Service Plans (IFSPs), Individual Education Programs (IEPs) and 504 Accommodation Plans. The EFMP also offers a respite care program, recreational activities, a summer camp and holiday celebrations. Workshops on issues relevant to military families with Exceptional Family Members are offered through the Specialized Training of Military Parents (STOMP) Program. The EFMP manager will assist with transition, relocation and continuity of care issues. Parents of children with special needs are encouraged to participate on the EFMP Committee as parent representatives. Sponsors and spouses provide valuable information and insight on the challenges and successes they experience over the course of the military career. The Army Volunteer Corps (AVC) program promotes quality experiences and environments which recognize the valuable contributions volunteers make to the Fort Leavenworth community. Every effort is made to match individuals and families with a volunteer position or project that is rewarding and meets the needs of the organization. The Army Volunteer Corps Coordinator at 684-2800 can provide specific information and referrals to organizations both on and off the installation.

Army Family Team Building (AFTB) is an Armywide educational self-development program for family members, which provides knowledge
and skills to promote personal growth, independence and self reliance. AFTB also sponsors the XO/S3 Spouse Seminars and an annual
Women’s Conference.

The Army Family Action Plan (AFAP) Conference is conducted annually to address well-being issues for local or Department of the Army action. The AFAP program provides a voice for Soldiers, spouses, teens, civilian employees, and retirees to elevate their concerns and recommend solutions to issues affecting everyone’s standard of living.

Family Readiness Programs provide training and assistance for setting up family readiness groups, sponsorship programs and rear detachments. Briefings are offered for mobilization and deployments and reunion briefs for both service members and their families. A Waiting Families Group is in place for families of members deployed or serving an unaccompanied tour.

Employment Readiness Program (ERP) offers job searches, resume writing, federal applications, local networking and job placement for family members and separating/retiring service members and DoD civilians.

American Red Cross

No one ever said life is easy on Soldiers or their families. Military life often creates unforeseen hardships. The good news is that the American
Red Cross Armed Forces Emergency Services can provide assistance. The daytime phone number is 684-4842 or 684-4843. For emergencies, phone (877) 272-7337.

Emergency Communications

In the event of an emergency, critical accident, illness or death in a service member’s immediate family, the Red Cross should be called at once.
The Red Cross is equipped to verify the situation and relay an emergency leave request, if necessary, to proper authorities. It is then up to the commanding officer to decide whether or not to grant leave. The Red Cross can also relay information about the birth of a child.

Emergency Financial Assistance

When an emergency arises that requires the presence of the service member or his or her family, the Red Cross may provide an interest-free loan or a grant for travel expenses. These funds, which may be authorized for other emergencies as well, are disbursed on the basis of need.

Red Cross Volunteers

People interested in volunteering with the American Red Cross should contact the Fort Leavenworth office at 684-4842. The American Red Cross Volunteer Office in Munson Army Health Center and staff are available to assist you with fulfilling a variety of volunteer opportunities at Munson and at the Veterinary Services Clinic. The Red Cross also encourages anyone interested in becoming a volunteer instructor for one of its many health and safety classes to contact the Leavenworth County Chapter at 682-6222.

Health and Safety Training

Service members and their families interested in health and safety training should contact the Leavenworth County Chapter for first aid and CPR instruction, aquatics and water safety training, swimming instruction and lifeguard training. Out of the pool, young people can better prepare for the responsibilities of being a babysitter through the chapter’s babysitting course. Other informational courses include F.A.C.T. — First Aid for Children Today — that provides comprehensive health and safety information for children kindergarten through third grade, B.A.T — Basic Aid Training — for children in fourth grade, and a disaster safety curriculum known as Masters of Disaster.

Army Substance Abuse Program

The Army Substance Abuse Program (ASAP) is at 614 Custer Ave. The program staff consists of the Alcohol and Drug Control Officer (ADCO), the Prevention Coordinator/Employee Assistance Program Coordinator (PC/EAPC), the Installation Bio-Chemical Testing (IBTC), and the Risk Reduction Program Coordinator (RRPC). The primary mission of ASAP is to support the command by providing substance abuse deterrence efforts, education, prevention programs.

An Employee Assistance Program Coordinator is available to all civilian employees, retirees and family members to address issues and concerns regarding alcohol/drug use and abuse. The EAPC provides people with a means to discuss and receive help for problems and conditions that are affecting their quality of employment and life.

The IBTC is the primary point of contact for all urinalysis issues.

The RRPC manages the Risk Reduction Program, which is a commander’s tool used to identify high risk units requiring intervention services. It is designed to decrease Soldiers’ high risk behaviors thus increase Soldier and unit readiness.

Medical Care

Munson Army Health Center provides health care through Primary Care Managers. Primary Care Managers are physicians, physician assistants, and nurse practitioners that are responsible for your primary care needs and coordinate your specialty care needs. To be assigned a Primary Care Manager, patients must enroll in TRICARE Prime.

Active-duty military are required to enroll when they in-process. Family members and others must choose to enroll. Those wishing to enroll or to transfer their enrollment from another duty location should stop by the TRICARE Service Center located in Munson Army Health Center or make the change by using the Triwest Web site, www.triwest.com.

Patients who choose not to enroll under TRICARE Prime may seek civilian medical care through either TRICARE Standard or TRICARE Extra (the preferred provider option). Cost shares and deductibles for both these programs are higher than fees in TRICARE Prime.

Munson does not have a traditional Military Sick Call. Those types of appointments are booked using the Advanced Access system.

Advanced Access - Gentry Health Clinic, the outpatient clinic, operates under advanced access scheduling. The advanced access approach means that when you call for an appointment your health-care needs are met during that telephone call. You will either be seen that day or within the next seven days at your discretion. The clinic does not schedule advance appointments or follow-up appointments. Patients can schedule follow-up appointments before they leave the clinic by using the appointment line telephone in the Gentry lobby. Therefore, when someone needs to see a provider for anything from acute illnesses to routine care, they must call on the day they want to come in. Appointments can be made by calling (913) 684-6250, or booked online at https://www.munson.amedd.army.mil.

Munson Army Health Center does not have an emergency room. Munson Army Health Center’s Primary Care Clinic, Gentry Health Clinic, is open daily, excluding Sundays and holidays, and can take care of minor emergencies. For problems that occur after the clinic closes, contact the physician on-call at 684-6000.

If directed to the emergency room, all beneficiaries are encouraged to use one the the three network emergency rooms in Leavenworth at Cushing Memorial Hospital, St. John Hospital and the Dwight D. Eisenhower VA Medical Center.

For major emergencies, immediately CALL 911.

Note: If you call 911 for an ambulance, one will be dispatched from a civilian ambulance service and you will be transported to a local emergency room for treatment.

Dental Care

The Thomas L. Smith Dental Clinic provides dental care to active-duty service members. The clinic is at 520 Pope Ave., between the Main Post Chapel and Munson Army Health Center. Clinic hours are from 7:30-11:30 a.m. and from 12:15-4:15 p.m., Monday through Friday. The phone number is 684-5516.

Dental exams for active-duty members are on a walk-in basis, Monday through Friday, 8:30-10:30 a.m., and from 12:15-2:55 p.m.

Dental Sick Call is Monday through Friday, 7:30 to 9 a.m. Follow-up appointments and subsequent appointments for active-duty personnel are given at the time of exam.

Active-duty family members, retirees and their family members are limited to standby care. This standby care is available only when an active-duty service member cancels or does not show for an appointment.

Active-duty service members are encouraged to enroll their family members in the TRICARE Active-Duty Family Member Dental Plan. Information regarding Tricare Retiree Dental Program can be found on their web site: www.ddpdelta.org or by calling (888) 838-8737.Emergency dental treatment is provided for trauma, uncontrolled bleeding, severe pain or infections for active-duty service members. Service members requiring emergency dental care during other than normal duty hours should report to Gentry Clinic during its operating hours. If treatment is required during hours that Gentry Clinic is not open, the service member should contact the duty officer at Munson Army Health Center at 684-6000. The on-call dentist will be contacted and arrangements will be made to render care.

Family members should use their civilian dentist for emergency care as provided by the TRICARE Family Member Dental Plan.

TRICARE - Active-Duty Family Member Dental Plan

The TRICARE Family Member Dental Plan (FMDP) has been an excellent dental program for family members, since the government pays much of the premium cost. Enrollment is voluntary and the active-duty member (also referred to as the “sponsor”) initiates enrollment of family members by contacting his or her Unit Personnel Action Clerk (PAC) and completing a DD Form 2494 or DD 2494-1. Once the DD Form 2494 or 2494-1 is completed, enrollment is entered into the Defense Eligibility Enrollment Reporting System (DEERS) and sent to United Concordia so that claims can be processed. Family members who are enrolled with the TRICARE Dental Plan can make appointments with any licensed TRICARE Dental Plan Provider they choose. More information can be found at www.ucci.com or by calling (888) 622-2256. For a complete list of participating providers near you, visit United Concordia’s Web site at www.ucci.com. Click on the “Provider Directory” icon and enter the city, state and specialty. This list is updated monthly. Smith Dental Clinic also has a list of participating dental providers.

If you are returning from an overseas assignment, sign up for TRICARE Dental Plan at your overseas personnel office within a month of departure. Enrollment will ensure that your eligible family members will be covered from the first day of eligibility. Call 680-4000 for more information.

Veterinary Services

The Fort Leavenworth Veterinary Treatment Facility is at 831 McClellan Ave. Hours for over-the-counter products are Monday through Friday, 7:30 a.m. to 3 p.m. Animals are seen by appointment only; call 684-6510. In the event of an emergency, consult the yellow pages for the nearest area civilian veterinarian.

Fort Leavenworth residents must register their pets at the Veterinary Treatment Facility within five days after arrival or acquisition. Post regulations require proof of current rabies vaccination and a microchip for registration. If you require either of these, an appointment can be made to complete the registration process.

Other services provided at the treatment facility include vaccinations, internal and external parasite control products, treatment of zoonotic diseases (diseases transmitted between animals and man) and the microchip identification program.

When preparing for overseas travel, the Veterinary Treatment Facility can provide answers to your questions about vaccinations,
quarantine requirements and all travel paperwork requirements. Keep in mind that this process could take 180 days or longer to complete
for some overseas destinations.

Adopt-a-pet: The Fort Leavenworth Stray Animal Facility is collocated with the Veterinary Treatment Facility and has animals for adoption. The adoption fee includes a microchip ID and rabies vaccination. Other services that can be performed at the time of adoption include a fecal check for cats and dogs, distemper and heartworm tests for dogs, and feline leukemia vaccination and testing for cats.

Household Hazardous Waste/Product Collection Point

The Household Hazardous Waste/Product Collection Point is located in the DIS Environmental Division at 820 McClellan Ave. Hours are
8 a.m. to 4 p.m., Monday through Friday. In addition to HHW/Products, used oil and antifreeze are accepted. Items that are reusable are available for free to anyone who can use them. These typically include paint, cleaning products, motor oil, wood stains and an ever-changing assortment of other products.

Adjutant General Services

The Adjutant General Division has offices in buildings 193 and 194 at 861 and 881 McClellan Ave. Personnel Services in building 193 are ID Card issue, DEERS enrollment and update, in/out processing, Military Personnel Records update, Casualty and Mortuary Affairs, Strength Management, Retirement and Transition Services, Database and PAI Management, and Soldier Actions. Appointments may be made through the Soldier’s respective G-1/S-1.

Mail and Distribution and Records Management, including Freedom of Information Act, are in building 194.

Legal Services

The Office of the Staff Judge Advocate provides legal assistance and claims support to Soldiers, retirees, and eligible family members. Located
at 415 Custer Ave., the OSJA is directly south of Otis Hall off Kearney Avenue (Otis Hall is across from the Beehive). Legal Assistance and the Claims Office are in the same building. General information concerning services, customer service hours and contacts is available by calling
684-4941 or by viewing the OSJA Web site.

The Legal Assistance Office advises clients on general legal problems including divorce, adoption, consumer problems, real estate deeds, estate planning and wills, landlord-tenant matters, military evaluation report appeals, and financial liability investigations. The Legal Assistance attorney cannot represent members of the Fort Leavenworth community in civilian litigation or provide advice on business activities, but the Legal Assistance Office does maintain a list of competent civilian attorneys.

Appointments for legal assistance may be made by calling 684-4944. Saturday appointments are also occasionally available depending on the availability of Reservist support.

Notary public service is available on a walk-in basis Monday, Tuesday, Wednesday and Friday 8 a.m. to 4:30 p.m. and Thursday noon to 4:30 p.m. Powers of attorney, including those required for medical care of family members, are also available. Notaries are also available at the Command and General Staff College, Munson Army Health Center, and the Command Judge Advocate, USDB, for personnel at those locations.

Active component Soldiers, who are not Kansas residents, are exempt from Kansas personal property tax. If you are not a Kansas legal resident, you can avoid paying the annual Kansas personal property tax on vehicles by completing an affidavit for tax exemption. These affidavits are only available at the Leavenworth County Treasurer’s Office. Details regarding the registration requirements are outlined earlier in this guidebook under the title “Vehicle Registration.”

Income Tax assistance is available during tax season (approximately Jan. 15 to April 15). Forms and instructions for federal and state taxes are available at the Tax Center. Also, the center can electronically file federal tax returns and many state returns. The Tax Center’s telephone number is 684-4986 (this telephone number is in use only during the tax season). Tax preparation is typically provided by appointment. A limited number of walk-in appointments may also be available.

Claims assistance is available on a walk-in basis Monday through Friday from 9 a.m. to 4 p.m. The Claims Office phone number is 684-4945.

Defense Military Pay Office

The Defense Military Pay Office (DMPO) is located in Bldg. 87 at 823 McClellan Ave. in the west end of Civilian Personnel Advisory Center. Hours of operation are 7:30 a.m. to 4:30 p.m., Monday through Friday. The Customer Service Section handles any military pay changes, advances of pay, Temporary Lodging Expense (TLE) claims or pay inquiries (684-5722), PCS or separation questions (684-5738) and PCS payments for advances and settlements for military employees (684-5710).

Transportation Services

The Transportation Office is at 450 Pope Ave., near the Garrison Headquarters. Hours are Monday through Friday, 7:30 a.m. to noon and
1-4:30 p.m. The telephone number is 684-5637.

Passenger Travel handles TDY, Passport, and Port Call, at 684-4067. Hours are Monday, Tuesday, Thursday and Friday 7:30 a.m. to noon and
1-4:30 p.m., and Wednesday, 9 a.m. to noon and 1-4:30 p.m. from Sept. 1 through Jan. 31. From Feb. 1 through Aug. 31, the hours are 7:30 a.m. to 4:30 p.m. Monday through Friday.

The Personal Property Shipping Office handles household goods and baggage shipments. Hours are Monday, Tuesday, Thursday and Friday, Sept. 1 through Jan. 31 from 7:30 a.m. to noon and 1-5 p.m., and Feb. 1 through Aug. 31 from 7:30 a.m. to 5 p.m. For automated information on these shipments call 684-5635. Shipping a privately owned vehicle (POV) is a personal responsibility, and individuals must complete all arrangements. The POV registration or title and seven copies of PCS orders will be needed at the port.

Information sheets with addresses and telephone numbers of available ports can be obtained in the Personal Property Section. The primary
port for Fort Leavenworth is Pontoon Beach, Ill. (near St. Louis). Prior approval must be obtained to use an alternate port. Submit a written

Army Continuing Education Services

The Army Education Center provides a wide range of educational opportunities designed to support the professional and personal development of Soldiers and their adult family members. Programs range from basic skills to graduate degree opportunities. Department of Defense civilians and retirees are also eligible to participate in many of the programs and services offered at the Education Center.

Educational services available include career and educational counseling, military and academic testing, traditional and independent study opportunities, information on sources of financial aid and assistance with evaluating military schools and experience toward civilian education
and jobs.

There are five regionally accredited institutions offering courses and programs on Fort Leavenworth: Kansas City Kansas Community College, Upper Iowa University, Central Michigan University, Kansas State University and Webster University.

Associate degree programs are offered in liberal arts, criminal justice/police science, criminal justice/corrections and general studies. Bachelor degree programs include business administration, management, public administration, social science, human services, technology and
information management, and criminal justice. Master’s degree programs include business management, human resources and general administration, information technologies management, procurement/acquisition, human resources management, adult and continuing education, and security studies.

A complete listing of the many colleges and universities within commuting distance of Fort Leavenworth is also available from the Education Center.
Tuition Assistance (TA) is provided for courses at the associate, bachelor and graduate degree levels for eligible Soldiers. Education counselors can provide information on multiple sources of financial aid, such as Army TA, VA educational benefits, PELL grants and scholarships.

The Education Center administers diagnostic, placement, aptitude, academic tests, and interest and skill surveys free of cost to active-duty Soldiers. A Learning Resource Center provides access to a wide variety of computer and video-based tutorials and instructional programs. Many of the materials are available for use in the center or to check out.

Participation in any voluntary education program or service begins by visiting with an education counselor. Counselors are available on a walk-in basis, Monday, Tuesday, Thursday and Friday from 7:30 a.m. to 4:30 p.m., and Wednesday 7:30 a.m. to 5:30 p.m. The Education Center is on
the lower level of the Family Support Center, 120 Dickman Ave., near the intersection of Grant and Kansas avenues. Visit or call for additional information at 684-2496.

Transcript Services (AARTS)

The Army/American Council on Education Registry Transcript System (AARTS) translates military training and experience into civilian language that is easier for colleges and potential employers to understand. It may be used to obtain college credit by providing learning institutions with credit recommendations based on the American Council on Education (ACE) Guide. The actual credit granted will depend upon school policy and the applicability of military training to the course of study.

Army Career and Alumni Program (ACAP)

Army policy requires all Soldiers to receive an Army Career and Alumni Program (ACAP) pre-separation briefing at least 180 days before separation/retirement. Soldiers planning to retire are authorized to initiate pre-separation counseling up to 24 months before retirement date,
and all others to initiate pre-separation counseling up to 12 months before separation date.

The ACAP provides outplacement training, career guidance, benefits information and job search assistance. Services are available to honorably transitioning Soldiers (active or Reserve), Army civilian employees, members of other DoD components (military or civilian), and family members of all the above.

The Civilian Personnel Advisory Center (CPAC) is in the east end of Building 87 at 821 McClellan Ave. It’s the center for information and advice about current employment opportunities. The CPAC manages the Fort Leavenworth personnel system for civil service and nonappropriated fund (NAF) positions and provides information and guidance on issues such as spouse preference, handicapped employment, veterans’ preference, and application procedures.

Civil Service positions most often available at Fort Leavenworth are information technology specialists, operations research analysts, and secretaries/clerks (office automation). There are limited opportunities for management analysts, registered nurses, education specialists and technical positions.

Nonappropriated Fund (NAF) employment opportunities are primarily in the clerical, trades and crafts occupations. Examples are automation clerks, clerks, school-age assistants, education aids (child care), custodial workers, cooks, and recreation assistants. The Fort Leavenworth NAF/CPAC announces vacancies and accepts completed applications.

Employment information on civil service vacancies is also available on the Internet at: www.cpol.army.mil and www.usajobs.opm.gov. Announcements are updated daily. The announcements contain information on eligibility and application requirements. The Web sites’ database for applicants offers a resume tutorial and self-nomination tracking system. If you do not have access to the Internet, the CPAC has a workstation for applicants to prepare and submit applications and for employment inquiries.
